Baptism
At least two weeks notice prior to scheduled
baptism. For immediate assistance,
please call the Parish Office.
- A Registration for Baptism Form must be
completed and submitted to the parish office at
least one week prior to the requested date of
baptism. Please call the Parish Office to request a copy of this form and
for other inquiries such as date availability and other preparation details.

when you visit us in the parish office.
- Baptismal Preparation Fee
Donation made payable to Cathedral Chapel, which includes
administration and priest stipend:
Private
- $250.00 (Parishioners)
$300.00 (Non-Parishioner)
Regular
- $100.00 (Parishioner/Non-Parishioner)
- Deposit
- $50.00 (2 weeks prior)
Fully paid (1 week prior)
Non-refundable 3 days prior to scheduled date

- Baptismal Requirements
- A copy of the child's birth certificate or hospital certificate
- Parents:
At least one parent must be a baptized and practicing Catholic.
A copy of the Baptismal Certificate of one parent (if available).
For Non-Parishioners, a letter from the Pastor of your parish
have child baptized at Cathedral Chapel.
- Godparents:
Must be a baptized Catholic and had also received the Sacrament
of Confirmation. Copy of Confirmation certificate (if available).
Letter from Pastor of his/her parish indicating person is an active
parishioner or practicing Catholic.
